Thank you for choosing Dr. Barber and our clinic, to provide Behavioral Health Services for yourself and/or your family member. This page has been set up for you in order to make the intake process as convenient as possible. The information requested is necessary to satisfy governmental regulatory, insurance company and healthcare requirements. The paperwork needs to be completed and returned to our office at the time of your first appointment.
You will find an instruction sheet and the required forms on the paperwork page, a map of our office location and contact information for your convenience. It is also strongly recommended that you call your insurance company to inquire about out patient mental health service benefits provided under your policy. (For example: deductibles, co-pays, and co-insurance.)
If you have a managed care plan (which you may no know until you call) you will need to let them know that you have made an appointment with Dr. Barber and verify that Dr. Barber is a network provider for your groups plan. Although we make every effort to check insurance information we can not guarantee the accuracy of the verbal information. Benefits are only verified after they are received in writing from the insurance company in the form of an explanation of benefits.
Things to remember to bring to your first appointment include;- Any test results and contact information from your primary care physician , other therapists or specialist doctors related to the reasons for seeking care with Dr. Barber
- Insurance card(s) and drivers license or state id
- Any custody/ adoption/ guardianship court orders for minor children
If the patient is a child having problems in school:
- Report cards
- Notes from teachers
- Copy of IEP's and any school testing
